Managing your health just got easier with the Kaiser Permanente Health Ally app, designed for patients enrolled in a diabetes or hypertension Remote Monitoring program. Once enrolled by a KP clinician, use this mobile app to seamlessly share device data with your care team.

If you aren’t enrolled and think you may qualify to participate, reach out to your care team for information.

• Sync and manage your device readings using your KP.org login
• Participate in multiple remote monitoring programs in one application
• Share your readings with your care team
• See the goals assigned by your clinician
• Have easy access to the Telehealth Support team

    Will not allow me to sign into app. I have no problem signing into website to view my account however, the app does not work.

    This app is too time consuming, always have to pair,etc this with the meter, sometimes three times a week. The one touch app was better, more reliable. Hope this helps !!
