Open the CH Care OnDemand app, request a visit and provide medical history. Within minutes, a board-certified clinician will review your medical history and contact you via phone or video for a consultation/visit. The doctor will recommend next steps and/or submit a prescription to your local pharmacy. NOTE: • Children under the age of 2 are not eligible to receive care through CH Care OnDemand • If you are a Catholic Health associate on the First Choice Healthcare Plan, dependent children between the ages of 18 – 26 can request care through CH Care OnDemand without an adult • At this time CH Care OnDemand isn’t offered internationally. If you are a Canadian associate of Catholic Health enrolled in the First Choice HealthCare Plan you are not able to use Telehealth services in Canada. If you are experiencing more serious symptoms, please go to your nearest ER for immediate treatment. These symptoms may include chest pain; uncontrolled bleeding; severe or sudden pain; coughing or vomiting blood; sudden dizziness, weakness, change in vision, slurred speech; severe or persistent vomiting or diarrhea; or changes in mental status, such as confusion.
